Valid photo id

  • Driver's license

  • Passport

  • U.S. military ID (active duty or retired military)

  • Permanent resident card

ORI #

The Digital address or Originating Agency Identification number for where the fingerprint results will be sent to.

Payment

We accept most forms of payments: Credit cards, Checks, Cash, money order, cashiers check, etc. We also accept company payment agreements.

 

If paying by check, a $50 fee will be charged for any returned check.

If you cancel within 24 hours of the appointment then a $50 dollar fee will be charged.

After cancellation, if you reschedule within 48 hours then a $25 credit out of the $50 will go back towards your fee.
